HEALTH AND IMMUNIZATION Complete Student Health Examination-HRS 3040 (Must include Tuberculin Skin Test or Tuberculin Screening with date and results.) “Yellow Form” Certificate of Immunization-HRS Form “Blue Card” Kindergarten, first grade, second grade, and third grade students are now required to have two MMR immunizations. Also, date of Varicella Disease or date of Shot is required. Part A-Student has received all required immunization or Part B-Temporary Medical Exemption (Additional certification must be presented on or before the expiration date) or Part C-Permanent Medical Exemption or HRS Form 681-Religious Exemption |
COPY OF SCHOOL RECORDS Report cards for students transferring from another school must be presented. EMERGENCY CONTACT CARD INFORMATION This must be completed, signed and dated by the parent/guardian. HOME LANGUAGE SURVEY CARD This must be completed, signed and dated by parent/guardian.
